What You’ll Do
As a Home Health RN, you’ll provide skilled, compassionate care while working independently and collaborating closely with physicians and an interdisciplinary care team.
Your responsibilities will include:
Completing comprehensive patient assessments, admissions, and ongoing care planning
Coordinating care with physicians and the interdisciplinary healthcare team
Delivering skilled nursing services, including therapeutic, preventive, and rehabilitative care
Monitoring patient progress and updating individualized plans of care
Educating and supporting patients and their families throughout their healthcare journey
Maintaining accurate, timely clinical documentation while ensuring compliance with regulatory standards
Identifying changes in patient condition and communicating updates to the appropriate members of the care team
Promoting patient independence, safety, and overall quality of life in the home setting
What You’ll Need
Current Registered Nurse (RN) license in the state of Tennessee
Graduate of an accredited nursing program
Minimum of one (1) year of clinical nursing experience (ICU, Medical-Surgical, or Rehabilitation preferred)
Home Health experience preferred, but not required
Current CPR certification
Valid driver’s license, reliable transportation, and current automobile insurance
