What You’ll Do
Provide physical therapy services according to a physician’s plan of care and state practice guidelines
Evaluate patient function and assist in developing individualized care plans
Educate patients, families, and caregivers on treatment plans and therapeutic activities
Translate prescribed exercises into functional activities and activities of daily living
Recommend modifications to equipment and home environments to support patient safety and independence
Collaborate with physicians and interdisciplinary team members to coordinate care and monitor patient progress
Document clinical notes, progress reports, and summaries in a timely and accurate manner
Support and motivate patients and families throughout the rehabilitation process
Supervise Physical Therapist Assistants in accordance with company policy and state regulations
Maintain accurate clinical records and track patient goals and outcomes
Qualifications
Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
Current Physical Therapist (PT) license in the state of practice
At least two (2) years of experience in an acute care or similar setting (preferred)
Current CPR and First Aid certification
Reliable transportation, valid driver’s license, and current automobile insurance
