Provide critical social work services at the Czorny Alzheimer Centre in Surrey, B.C. as a Casual Social Worker. Focus on delivering compassionate therapeutic counseling.
In this role, you will collaborate with healthcare professionals while reporting to the Health Services Manager. Your responsibilities will include conducting psycho-social assessments, delivering clinical counseling to residents and families, and leading educational initiatives. Emphasize improving resident outcomes through effective care planning and service delivery in the community.
Key Responsibilities:
• Conduct assessments to evaluate residents' needs
• Work collaboratively with the interdisciplinary team
• Offer educational seminars on adaptation to illness
• Document resident progress and maintain records
• Engage in community outreach to enhance care
Requirements:
• Master’s Degree in Social Work from an accredited institution
• BC College of Social Workers registration required
• One year related experience or equivalent training
• Strong analysis and communication skills
• Experience in community resource referral
