Meaningful Work That MattersBuild relationships that change livesHelp individuals achieve personal goalsSupport independence and self-confidenceMake a positive impact every dayBe part of a mission-driven organization dedicated to serving othersWhat You'll DoThe Assistant Program Coordinator will:Ensure a meaningful day for consumersOversight of 1 CLAWeekly oversight of CLAAlways ensure cleanliness of CLA; confirm cleaning charts are being completedConduct Monthly fire drillComplete Program Oversight as assignedConduct quality inspections on all shiftsProvide assistance with emergencies at the CLASupervise and direct the work of Residential Specialists to include the following:Medical Appointments/ medication management (in conjunction with the Program Coordinator)Attend all consumer medical appointmentsComplete requirements to become a Medication Administration ObserverMonitor medications for refills and reconcile weekly medications with the PC upon delivery prior to the start of each weekEnsure staff administering medications have completed, dailyEnsure staff are current in medication administration trainingCheck-in cycle medications (1st check)/ complete medication changesOn Call ResponsibilityParticipate in on-call rotation Be accessible via telephone twenty-four (24) hours per day, seven (7) days per week for assigned podPlan with another Program Coordinator/Assistant Program Coordinator for call coverage if unavailable for callsPART team -
Participate in PART team and associated trainingsTraining Monitor compliance list and inform staff of upcoming training needsConduct and observe medication practicums for employeesConduct in home orientation and individual specific training for substitute staff at Program Coordinator directionReview Critical Information Book with all employees working in the CLAProvide documentation for all training to the Training DepartmentComplete understanding of applicable regulations, bulletins, and lawsMaintain home according to applicable regulations, bulletins, laws, and agency policyMaintain consumer files and records according to regulations, bulletins, laws, and agency policyMaintain staff training according to regulations, bulletins, laws, and agency policyReview all CLA EMR (SETWORKS) documentation dailyElevate any concerns to immediate supervisorReview documentation and ensure completionFollow up with staff regarding any missing documentationTrack completion of RS daily chore charts (Day/evening & Awake night)General SupervisionUtilize Person Directed Framework/Non-Violent Communication during interactions with consumers and employeesMentor and provide ongoing feedback and indicate areas of improvement regarding job performancePartner with consumers and employees to identify issues and resolve problemsComplete accurate and timely employee performance evaluations prior to the due dateReport all employee concerns to the Program Coordinator and the Human Resource DepartmentComplete food shopping and maintain food supply in the CLAElevate any issues/needs to Program Coordinator Monitor proper completion of all required paperwork and SETWORKS documentation; submit all items in accordance with established deadlines, Monitor completion of MARS/ EMAR dailyAssist with vehicle training and wheelchair securement training for employees working at the CLA and operating the vehicleComplete expense records for house petty cash and consumer funds based on receipt content and forward to Program CoordinatorEnsure that all house and individual funds are managed and maintained safely and properly.
