HarmonyCares seeks an Advanced Practice Provider to deliver home-based primary and palliative care to medically complex, home-limited patients. In this role, you will conduct in-home and virtual visits, perform comprehensive assessments, manage chronic and acute conditions, and coordinate care with an interdisciplinary team. You’ll develop patient-centered care plans, support advance care planning, and work closely with families and caregivers.
