Required Qualifications

Experience
Minimum

2 years of Acute Medical Care Management experience
Experience performing:

Care Coordination
Patient Assessment
Discharge Planning
Treatment Planning

Experience must be

recent, within the last 3 months
Prior travel nursing experience required ( Hard Stop )
Acute care hospital experience required
Charting
Epic experience required (Hard Stop)
Licensure
Active California RN License required
