Local Travel Home Health Physical Therapist for a 13‑week contract in Springfield, Missouri. Provide comprehensive patient evaluations, develop individualized treatment plans, and manage OASIS documentation in a home health setting. Earn up to $2,700 per week while delivering compassionate care across the region.

Key Responsibilities:

Conduct comprehensive patient evaluations in the home health setting

Develop individualized treatment plans focused on mobility, strength, balance, and independence

Perform start‑of‑care assessments, recertifications, and routine home visits

Complete accurate OASIS documentation and maintain compliance with regulations

Serve as case manager coordinating care with physicians, nurses, therapists, and interdisciplinary teams

Educate patients and caregivers on therapeutic exercises, mobility strategies, and safety precautions

Monitor patient progress and modify treatment plans as needed

Accurately document all interactions and outcomes using Matrix EMR

Qualifications:

Doctorate or Master’s degree in Physical Therapy from an accredited program

Active Missouri PT license required

Strong experience with OASIS documentation, case management, start of care assessments, and recertifications required

Excellent communication, organizational, and time management skills

Ability to work independently in a home health environment

Preferred Skills:

Current CPR/BLS certification preferred

Home health experience highly preferred for travel assignments

Experience with Matrix EMR preferred
