We are currently collaborating with a thriving home health organization in search of a dedicated and caring Registered Nurse (RN) to join their team on a full-time basis. This role is well-suited for an RN who thrives in a role that offers autonomy, case management responsibilities, and the opportunity to forge meaningful connections with patients within a home care environment.
Job Description

The RN will be responsible for delivering expert nursing care to patients residing in

Aurora, Central Denver, and the Littleton area.
The role encompasses a variety of tasks, including:
Commencing patient care visits
Conducting routine skilled nursing visits
Handling recertifications
Managing discharges
Ongoing case management
The work schedule for this position is Monday through Friday during daytime hours.

Medication management

Coordinating with therapy services
Providing patient education and devising care plans
