Physical Therapist - Travel Contract

Minimum Requirements
Bachelor‘s, Master‘s or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
Active State License is Required to Start the Assignment
BLS Certification May Be Required from AHA or ARC
CA license required
BCLS required
HCHB required
1 year of recent Skilled Home Health experience within the last 3 years required
OASIS experience required
Own transportation required

Assignment Details
Monday through Friday, 8:00 AM to 5:00 PM
Geriatric caseload
May visit up to 6 patients per day
30 productivity points standard
Occasional travel radius of 50+ miles
1:1 patient ratio
