Minimum Requirements
Bachelor‘s, Master‘s or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
Active Indiana State Physical Therapy License is Required to Start the Assignment
BLS Certification from American Heart Association or American Red Cross may be required
Outpatient clinic experience required
Experience evaluating and developing treatment plans, including functional treatment programs and goal-oriented care
Competence with outpatient documentation, goal writing, letters of medical necessity, and charge entry
Ability to supervise and provide direction for discipline-specific therapy services and to participate in student training
Preferred: Prior experience working in a Select Medical or similarly governed clinic environment
