Role description

The Connecticut Institute for Communities seeks a Mental Health and Disabilities Services Professional to provide integrated support to individuals and families in underserved communities. In this role, you will conduct assessments, develop person-centered care plans, and deliver counseling and rehabilitative interventions for clients with mental health needs and disabilities. You will coordinate with healthcare, educational, and social service providers to ensure comprehensive care, connect clients to vital community resources, and advocate for accommodations and benefits.
