Southwest Ohio
Schedule:
Monday–Friday, 8:00 AM – 5:00 PM (Only 2 holiday on-call days per year)

Key Responsibilities
As a
Hospice RN Case Manager , you will:
Manage a
patient caseload
and lead the full nursing process: assessment, admission, care planning, implementation, and evaluation
Deliver
holistic patient care , addressing physical, emotional, psychological, social, and spiritual needs
Collaborate with the
interdisciplinary care team , including physicians, social workers, and chaplains
Coordinate care with attending physicians and external providers
Conduct
routine patient visits
and update individualized
plans of care
Provide
end-of-life care , including clinical pronouncement and family support
Maintain
accurate, timely documentation
in compliance with state, federal, and company standards
Supervise and oversee care delivery by
Home Health Aides (HHAs)
Qualifications
Active
Registered Nurse (RN) license
in Ohio (or compact state if applicable)
Graduate of an
accredited nursing program
Current CPR and First Aid certification
Knowledge of
quality assurance and hospice care standards
Valid
driver’s license
and
auto insurance
Preferred Skills
Hospice care, palliative care, RN case manager, patient assessment, care coordination, EMR documentation, interdisciplinary team collaboration, end-of-life care, home health, clinical nursing, patient-centered care.
